HMO Collaboration Plans
Collaboration Plans tell what health maintenance organizations will do to meet identified public health goals for communities in the areas they service. Local public health units and other community organizations in the health maintenance organization's service area provide input to the collaboration plan.
Minnesota Statute 62Q.075 requires that collaboration plans be filed with the Commissioner of Health and describes the filing requirements, the information to be included in the plan, and the details of the review process. The Minnesota Department of Health, Public Health Practice Section receives these plans.
The 2020-2024 collaboration plan is a combined plan compiled by the Minnesota Council of Health Plans (MCHP). Health plan members include Blue Cross and Blue Shield/Blue Plus of Minnesota (BCBS), HealthPartners (HP), Medica, Hennepin Health, Sanford Health Plan of Minnesota, and UCare.
MDH is currently collecting and reviewing the 2024 - 2028 collaboration plans. They will be posted here when available.
