- Agency for Healthcare Research and Quality (AHRQ) offers free tools to help prioritize concerns and maximize interactions between providers, patients, and families. One tool is:
- Guide to Improving Patient Safety in Primary Care Settings by Engaging Patients which promotes meaningful engagement with patients and families.
- Family Voices is a national family-led organization of families and friends of children and youth with special health care needs (CYSHCN) and disabilities. One of their tools is:
- Family-Centered Care Assessment for Families (PDF) assesses strengths and opportunities for improvement.
- IncludeAlways is a patient and family engagement model developed by the Minnesota Hospital Association to include patients and families at every level throughout the health care system.
