Instructions for Local Public Health Act Annual Reporting

  • Performance Measurement Instructions for 2025 (PDF): These instructions will help you enter performance measures data into the REDCap reporting system for Local Public Health Act (LPH Act) annual reporting for calendar year 2025. Reporting for 2025 will open mid-February 2026 and must be completed by March 31, 2026.
  • Finance and Staffing Instructions for 2025 (PDF): This data provides an annual, system-level snapshot of workforce and expenditures, and provide a view of trends over time across the local public health system in Minnesota. Review these instructions before entering data into REDCap. Please share these instructions with everyone on the community health board who has a role in annual reporting on Local Public Health Act finance and staffing.
  • Performance-related Accountability Requirement Instructions (2025): For 2025, community health boards are required to demonstrate meeting Measure 2.2.5 Maintain a risk communication plan and a process for urgent 24/7 communication with response partners. Community health boards will submit this information via the Response Sustainability Grant reporting in REDCap in March 2026. The Center for Public Health Practice will work with the Center for Emergency Preparedness and Response to collect this information. For details about what community health boards will need to submit, visit the link above. CHS administrators should review what was submitted for the performance-related accountability requirement (PRAR) and will be asked to complete the validation form in the REDCap performance measure project certifying the information entered for PRAR in addition to what was entered for performance measures. 
  • Validation: If you are a CHS administrator, remember to complete validation to certify the data entered is accurate and true. There will be two validation forms for the CHS administrator to complete - one for finance and staffing, and one for performance measures and performance-related accountability requirement.
