Rural Health Transformation Program: Rural Clinical Fellowship for Advanced Practice Providers and Other Clinicians

Eligible Applicants:
- Accredited clinical training programs within accredited institutions of higher education in Minnesota. Eligible training programs include;
- Advanced Practice Registered Nursing
- Certified Nurse Midwifery
- Dental Therapy and Advanced Dental Therapy
- Mental health professional training programs in Alcohol and Drug Counseling, Counseling, Marriage and Family Therapy, Psychology, and Social Work
- Physician Assistant
- Clinical practice settings located in rural Minnesota communities, including:
- Certified Community Behavioral Health Clinics (CCBHCs)
- Community Mental Health Centers (CMHCs)
- Federally Qualified Health Centers (FQHCs)
- Health systems
- Hospitals
- Long-term care facilities
- Other clinics
- Private clinical practices
- Tribal clinical settings
The Rural Health Transformation Program Activities are supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $193,090,618.14 with 100 percent funded by CMS/HHS.
